2. Fill in all the blanks in the Buyers Club Member Application. This document requires your Membership ID. You may use any set of 9 digits. In order to assure uniqueness, you may use your Social Security number or as another suggestion, you may use the last 4 digits of your phone number plus the first 5 digits of your Social Security number. Specify a method of payment for $9.95 processing fee and your initial order, and sign it. Make sure to enclose a proof of your age.
3. Remember that it is your responsibility to provide Stone Tobacco with the necessary information to enable its staff to process your application. If this information is not provided, your application will NOT be processed.
